| Smith Reporting $25 Visa Gift Card Promotion |
1. Offer: Receive a $25 Visa Gift Card (Gift Card) for each qualifying deposition scheduled
through Smith Reporting by calling 615-256-2223 or by scheduling through www.smithreporting.com through December 31, 2009 (Entry Period).
2. How to participate: Individuals may receive a Gift Card by scheduling a qualifying
deposition directly through Smith Reporting during the Entry Period.
3. Qualifying Depositions: Qualifying depositions are Smith Reporting Depositions
scheduled for general litigation purposes and completed in full through job invoicing. Please note: Multiple depositions within the same Smith Reporting assignment number will award
only one Gift Card.
4. Fulfillment: Please allow two weeks after invoicing of the qualifying deposition for Gift
Card delivery.
5. Gift Card Recipient obligations: Gift Card Recipient (Recipient) agrees to abide by these
Official Rules and by the decisions of Smith Reporting which are final and binding on all
matters pertaining to this offer. It is the Recipient's responsibility to ensure that the Gift
Card is received, redeemed and/or distributed in accordance with the Recipient's company
policies. Smith Reporting assumes no liability should the Recipient's reciept, use and/or
distribution of a Gift Card violate Recipient's corporate policies. Smith Reporting assumes
no liability for lost, stolen or damaged cards.
6. By accepting the Gift Card, the Recipient agrees to the terms and conditions set forth by
Visa and that Gift Cards will be used in accordance with the Visa Gift Card
terms and conditions.
7. Promotion Changes: Smith Reporting reserves the right to change or discontinue this
promotion at any time for any reason or for no reason.
